Nurse Manager of Hospice Home |

Since 1994, the Hospice Home in Burlington has provided loving, compassionate care to our community. The Hospice Home is part of the Hospice and Palliative Care Center of Alamance-Caswell, a not- for-profit, dually certified agency that has cared for our community since 1982. Due to the impending retirement of our current nurse manager, an outstanding opportunity is available for a qualified registered nurse to manage the 16 bed facility, its staff, clinical operations - and maintain and improve the outstanding reputation we have earned. The Hospice Home Nurse Manager has overall responsibility for operations of the facility. This includes scheduling, supervision, supply ordering, problem-solving, interfacing with families, policy and procedure oversight, and compliance with state, federal and accrediting regulations. In addition, direct patient care within the paramaters of the N.C. Nurse Practice Act is included, such as patient assessment, care planning, teaching, performing nursing interventions, and support to patients/ families. As part of the agency's management team, you will have the opportunity to participate in the overall agency's management. The position reports to the Hospice Director of Nursing. QUALIFICATIONS: A current R.N. license and clinical experience of at least five years is required. Hospice experience and previous clinical supervisory experience is highly preferred; CHPN certification is required within 12 months of employment; a BSN is preferred

Provide Physical Therapy services for the agency’s Home Health and Hospice patients primarily in Alamance-Caswell area. Duties include Case Management, supervision of PTA and Home Health Aides as it relates to the patient’s plan of care, adherence to orders as well as agency policies and procedures, licensure standards and home health regulations. Maintains a working knowledge of reimbursement, documentation and related issues. Flexible schedule with some meeting and holiday requirements. Bi-lingual a plus! Please apply by August 27, 2010.

Assist the physical therapist in the provision of physical therapy. Provide physical therapy services to patients of all ages who have impairments, functional limitations, disabilities, or changes in physical function and health status resulting from injury, disease or other causes. Work under the supervision and direction of a physical therapist. Must have current PTA license with previous experience; excellent written and communication skills. Bi-lingual a plus.

The occupational therapist provides all occupational therapy functions in accordance with the patient's physician's written orders and in coordination with Agency staff in fulfilling requirements in the designed plan of care. These functions include the planning and administering of medically prescribed occupational therapy treatment programs for patients to restore function, relieve pain, and prevent further disability due to disease, injury or loss of body part. Duties will include: Treating patients in accordance with the plan of care to promote wellness, relieve pain and maximize performance in ADL/IADL using occupational means, patient/caregiver training and environmental modifications. The Occupational Therapist brings specialized knowledge enhancing the outcomes for particular patient conditions, educating agency and participating in program development for under-served populations. Bi-lingual a plus!
